A follow-up assessment for employees with existing restrictions or a prior occupational report, to determine whether circumstances have changed.

When It's Appropriate

Use This Assessment When

Previous restrictions were time-limited and their expected duration has passed.
An employee's condition or functional status has changed since the last assessment.
A prior report recommended reassessment at a future point.

What the employer supplies

The prior occupational report or restrictions on file
The current occupational question (e.g. are restrictions still required?)

What the physician may assess

Change in functional status since the last assessment
Whether previous restrictions remain appropriate

What the employer receives

An updated occupational report
A clear statement of whether restrictions are maintained, modified or removed

What is generally not disclosed

The employee's complete medical chart
Unrelated diagnoses, medications or medical history

Reassessment may maintain, modify or remove previous restrictions, depending on the physician's findings at the time of reassessment.
